Workup

  • Joint MRI to rule out TMJ pathology
  • Sleep questionnaire (negative for OSA)

Plan

Surgery-first protocol: Le Fort I maxillary advancement 5mm + BSSO mandibular setback 4mm + 3mm sliding genioplasty. Pre-surgical orthodontic phase shortened to 4 weeks for archwire insertion only.

Surgery

Bimaxillary osteotomies with patient-specific titanium plates designed from a virtual surgical plan. Theatre time 4h 20m. One-night admission. Inter-maxillary fixation avoided thanks to rigid custom plating.

Outcome

64-year-old male, 18 years of upper denture failure and progressive bone loss. Conventional implant placement deemed impossible without 12+ months of staged grafting.

Workup

  • Cone-beam CT (low-residual ridge confirmed)
  • Cardiology clearance (well-controlled hypertension)
  • DEXA scan — moderate osteopenia, not osteoporosis
  • Sinus and nasal endoscopy
  • Smoking cessation 6 weeks pre-surgery

Plan

4 zygomatic implants (Nobel Biocare) + immediate fixed temporary acrylic prosthesis same day. Final zirconia hybrid bridge at 4 months.

Surgery

Surgery under general anaesthesia, theatre time 3h 15m. All four implants achieved >40 Ncm primary stability. Patient ate soft food the next morning.

Outcome

Final zirconia bridge delivered at month 4. 12-month CBCT shows preserved sinus and ridge architecture. Patient sleeps without removable prosthesis for the first time in 18 years.

Incidental panoramic finding — 3.2cm radiolucent lesion in the right body of the mandible, displacing roots #45–#47. Asymptomatic.

Workup

  • Cone-beam CT for lesion mapping
  • Aspiration biopsy (negative for malignancy)
  • Vitality testing of adjacent teeth
  • Panoramic and lateral cephalometric review

Plan

Enucleation under local anaesthesia + IV sedation. Bony cavity grafted with autogenous chin block + xenograft mix and PRF membrane. Implant rehabilitation deferred 6 months.

Surgery

75-minute outpatient procedure. Histology confirmed odontogenic keratocyst. Patient discharged the same evening with prescribed analgesia and antibiotic prophylaxis.

Outcome

Bone fill complete at 6 months on CBCT. 2 implants placed at month 7 with porcelain-fused-to-metal bridge at month 10. No recurrence at 24-month follow-up.

  • Stress, sleep and screen-time history
  • EMG of masseter & temporalis

Plan

Stabilisation splint worn nightly + 8 weeks of supervised physiotherapy + 2 sessions of trigger-point injections to masseter. Occlusal selective grinding deferred.

Surgery

No surgery required. Botulinum toxin reserved as second-line if symptoms returned beyond month 6.

Outcome

Pain VAS dropped from 7/10 to 1/10 at week 12. Patient returned to full work hours. 24-month follow-up: 1 brief flare during high-stress quarter, resolved with splint compliance.
